Job Description & Requirements Specialty: Care Manager Discipline: RN Start Date: 09/14/2026 Duration: 13 weeks 40 hours per week Shift: 8 hours, days Employment Type: Travel Schedule: 8:00 AM – 4:00 PM Hospital experience required, including experience with discharge planning and placement in rehabilitation facilities Minimum 2 years of experience with acute inpatient hospital discharge planning Rotating weekends: Every 4th or 5th weekend Holidays: One summer and one winter holiday Experience with subacute patients required Day 1 Dental, Health, Vision insurance.
