Weekend Hospice Registered Nurse

About the Role:
We are seeking a dedicated and compassionate Weekend Hospice Registered Nurse to join our team in Chicopee, Massachusetts. As a member of our team, you will provide exceptional patient care and support to our patients and their families on weekends and during on-call rotations. This is a unique opportunity to make a meaningful difference in the lives of those we serve while working closely with a collaborative and supportive team.
Responsibilities:
Provide skilled nursing care to patients in their homes, hospitals, and hospice facilities
Develop and implement individualized care plans in collaboration with the interdisciplinary team
Conduct patient assessments, identify needs, and coordinate services to ensure continuity of care
Communicate effectively with patients, families, and healthcare providers to ensure timely and effective treatment
Providing 24/7 support to patients and families
Collaborate with the healthcare team to ensure seamless transitions and quality care
