Who We Are

Since 2004, The Gathering Inn has compassionately reached out to individuals experiencing homelessness, aiming to welcome new beginnings and transform lives. Originally a winter-only shelter serving 40 individuals nightly, we have evolved into a multifaceted organization with multiple locations, serving nearly 500 individuals and families daily. Our mission is to meet people where they are, inspire hope, and walk alongside them on their journey to sustainable housing. Through our emergency shelter, permanent supportive housing, and medical respite care, we provide programs and support designed to empower guests toward self-sufficiency and long-term stability.

What You’ll Do

As a Case Manager, you play a vital role in helping guests move toward stability and self-sufficiency. You will:

  • Conduct comprehensive assessments of guests’ needs, strengths, and goals through initial intake, and develop individualized care plans in collaboration with guests
  • Maintain accurate and up-to-date records of guest interactions, services provided, and outcomes, utilizing the Homeless Management Information System (HMIS)
  • Coordinate access to service providers and resources, ensuring eligibility, follow-through, and seamless delivery of support
  • Work collaboratively with community partners to advocate for guests, schedule appointments, arrange transportation, and make referrals
  • Facilitate and lead assigned classes or programs for guests
  • Uphold guest confidentiality and data privacy standards
  • Address behavioral concerns calmly and respectfully, maintaining safety and harmony
  • Participate in staff meetings, in-service training, and other professional development opportunities, including De-Escalation, Motivational Interviewing, CPR, and First Aid

  • Experience with homelessness, mental health, or substance use challenges is highly value
Requirements

Minimum Qualifications

  • A bachelor's degree with one year experience or an equivalent blend of educational qualifications and practical experience is necessary. This could include a high school diploma and a minimum of five years of relevant experience. Alternatively, candidates with an associate's degree and three years of experience will also be considered.
  • Computer skills for documentation and record-keeping.
  • Ability to pass a Live Scan background check
