Registered Nurse – Home Health

Schedule: 8 hours

Assignment Details:

• Contract Length: 14 weeks
• Guaranteed Hours: 40

Requirements:

• Current state license as a Registered Nurse
• Homecare Homebase Software Experience
• OASIS Experience
• 1-2 years of experience as a Registered Nurse in a clinical care setting
• 2+ years of home health experience
• CHPN certification (preferred)
• Proof of current CPR certification
• Current driver's license
• Reliable transportation and insurance

Roles & Responsibilities:

• Assess home health and/or hospice patients to identify their physical, psychosocial, and environmental needs.
• Implement, develop, document, and evaluate the Plan of Care (POC) to ensure quality and continuity of care.
• Educate patients and their families regarding care, diet, medications, and home safety.
• Supervise and provide clinical direction to support staff to ensure quality and continuity of service.
• Monitor cases for compliance with requirements of third-party payers.
• Participate in interdisciplinary team meetings and other necessary meetings.
