Weekend Registered Nurse

Role Overview Become a part of our caring community. As a CenterWell Home Health nurse reporting to the Clinical Manager, you will provide personalized, one‑on‑one care that helps patients regain independence in the comfort of their homes. Working with a dedicated team of physicians and clinicians, you will manage care plans that support recovery and help patients return to the life they love. Schedule Candidate must be available to work on Saturday, Sunday, and two additional days during the week. The two additional days will be determined with the candidate based on their availability. Scheduled weekly hours: 32 hours. Responsibilities Provide skilled nursing care to patients onsite in their homes, including administering medications, performing wound care, and conducting patient assessments. Develop and implement individualized care plans in collaboration with physicians and other healthcare professionals. Monitor patients' conditions and report changes. Educate patients and their families on disease management, medication, and treatment options. Maintain accurate records of patient care and coordinate with other healthcare professionals. Report patient care and condition progress to patient's physician and Clinical Manager. Oversee the work of Home Health Aides and monitor their appropriate completion of documentation. Required Qualifications Diploma, Associate, or Bachelor's Degree in Nursing with experience in Med Surg, ICU, ER, or acute care. Current and unrestricted Registered Nurse licensure. Current CPR certification. Experience collaborating with a team of healthcare professionals. Valid driver's license, auto insurance, and reliable transportation. Preferred Qualifications One year of nursing experience. Home Health experience. Pay Range Pay per visit/unit: $63.00 - $88.00. Base annual pay: $92,600 - $127,400. Additional Information Tuberculosis (TB) screening program: this role is patient‑facing and requires TB screening if selected. Driver safety program: requires valid state driver's license and personal vehicle liability insurance meeting state minimum limits or $25,000 bodily injury per person/$25,000 bodily injury per event/$10,000 property damage.
