Registered Nurse (RN)

Join our dedicated team providing essential home health care services. We are seeking a Registered Nurse (RN) to deliver high-quality patient care in a variety of home settings.

Responsibilities

• Conduct routine patient visits, focusing on wound care, wound vac management, and ostomy care.
• Manage care for patients with cardiac conditions, including congestive heart failure (CHF) and pneumonia.
• Administer IV antibiotics and provide care for patients with PICC lines.
• Perform routine lab draws as needed.
• Document patient interactions and care in our electronic health record system, HomeCare HomeBase.
• Collaborate with a team of health professionals including PT, OT, and social workers.

Patient Demographics

• Post-surgical wound care
• Cardiac care (CHF, pneumonia)

Work Schedule

This is a Monday through Friday position, with working hours from 8 AM to 5 PM and a one-hour lunch break. There are no weekend or holiday shifts required.

Qualifications

• Active Registered Nurse (RN) license.
• Current Basic Life Support (BLS) certification.
• Valid driver's license and insurance.
• Experience with HomeCare HomeBase EHR.
• Knowledge and experience in wound vacs, ostomy care, and general wound care.

Orientation and Training

All new hires will receive orientation specific to home health care, with a designated preceptor to assist during the onboarding process. Orientation will be completed within one week.

Additional Information

This position does not involve case management or patient scheduling. Travel is required to visit patients within their homes, using a provided Samsung tablet for charting and a cell phone for communication.
