Hospice - Registered Nurse IPU Full Time Direct Patient Care Columbus, OH, US Registered Nurse (RN) - Hospice House/Inpatient Unit Schedule Employment Type: Full-time Shift: 6:30 PM - 7:00 AM (Night Shift) Location: Inpatient Unit at OSU Medical Center Weekend Rotation: Every third weekend required Holiday Rotation: Required About the Role Join Ohio's Hospice as a Registered Nurse (RN) on our Hospice House/Inpatient Unit team, where you'll provide compassionate, patient-centered care to individuals living with life-limiting illnesses. You'll support patients and their families throughout their hospice journey while collaborating with an interdisciplinary team dedicated to comfort, dignity, quality of life, and clinical excellence. Key Responsibilities Provide skilled nursing care using the nursing process, including comprehensive assessment, care planning, implementation, and evaluation. Assess and address patients' physical, emotional, psychosocial, and spiritual needs while promoting comfort, dignity, and quality of life. Develop, implement, and regularly update individualized plans of care in collaboration with patients, families, and interdisciplinary team members. Monitor patient conditions, recognize changes in status, and promptly communicate significant findings to physicians and other healthcare providers. Evaluate and adjust nursing interventions, medications, and care plans based on changes in patient conditions and clinical needs. Participate in patient admissions, interdisciplinary team meetings, care conferences, and ongoing care coordination. Maintain accurate, timely, and thorough documentation of assessments, interventions, physician orders, medications, and patient status in the electronic medical record (EMR). Provide clear, effective patient handoffs using SBAR and other standardized communication techniques to support continuity and safety of care. Provide guidance, supervision, and support to personal care specialists, volunteers, and other caregivers to ensure care is delivered according to established plans and standards. Collaborate with physicians, nurses, support staff, and other departments to promote high-quality, patient-centered care. Participate in quality improvement initiatives focused on patient safety, clinical outcomes, regulatory compliance, and organizational excellence. Serve as a preceptor and mentor to new team members, providing clinical guidance and support during orientation and training. Qualifications Current Registered Nurse (RN) license in good standing, without disciplinary actions. Current CPR certification. Certified Hospice and Palliative Nurse (CHPN) certification preferred. Support is available for eligible employees pursuing certification. Strong clinical assessment, symptom management, and care planning skills. Experience documenting patient care in an electronic medical record (EMR) system. Valid Ohio driver's license, reliable transportation, and proof of auto insurance.
