Center Nurse: Chronic Care & Care Coordination
Registered Nurse
Oak Street Health in Columbus, OH is seeking a Center Nurse to join a value-based care team delivering protocol-driven chronic disease management, wound care, vaccines and patient education. You will coordinate with the care team to reduce hospitalizations and ensure high-quality, patient-centered care. The role reports to the Practice Manager, requires active RN licensure, BLS/CPR, EMR experience, and strong communication.
