Schedule Day Shift: 5x8-Hour (08:00 - 17:00) Job Description We are seeking a dedicated Home Health Registered Nurse to provide high-quality patient care in a home setting. The ideal candidate will have experience in skilled home care and be able to perform various assessments and interventions to promote recovery and rehabilitation of patients. Responsibilities Conduct patient assessments and develop individualized care plans. Administer medications and treatments as prescribed. Educate patients and their families on care protocols and wellness strategies. Monitor vital signs and report changes in patient conditions. Utilize adaptive devices and assistive technologies to improve patient mobility and safety. Document patient progress and maintain accurate medical records. Coordinate with interdisciplinary teams to optimize patient care. Required Experience / Certifications / Licensure Minimum of 1 year of home health experience required. BLS certification is mandatory. State nursing license or compact license required. OASIS training preferred. Experience with electronic charting systems (Cerner preferred). Valid driver's license and reliable transportation for home visits. COVID-19 vaccination required or valid exemption.
