Care Navigator

As a Care Navigator, you will serve as the primary point of contact for new business development, referral management, and community outreach. You will represent our hospice services to facility staff, attending physicians, hospitals, patients, and families, ensuring they understand our compassionate approach to end-of-life care. Your role will include public relations, education, and fostering relationships between our hospice team and the communities we serve. The coverage area will include: Clemson, Seneca, Pickens, Anderson, OconeeKey ResponsibilitiesIdentify, develop, and maintain relationships with referral sources to drive patient admissionsImplement and contribute to strategic marketing and outreach initiativesProvide education on hospice services and end-of-life care to healthcare providers, facilities, and the communityMaintain accurate records, including sales call logs, referrals, and expense reportsAddress service concerns, resolve issues promptly, and provide exceptional customer serviceAdapt to evolving business needs and demonstrate professionalism in difficult situationsExhibit strong communication, negotiation, and public speaking skillsWork independently with a self-driven, results-oriented approachEducationBachelor's degree in a relevant field preferred.ExperienceMinimum 3 years of successful outside sales experience in healthcare, hospice, or a related industryProven ability to develop and maintain referral networksStrong knowledge of hospice services and the ability to educate referral sources on benefitsSkills and AbilitiesExcellent verbal and written communication, including public speaking and presentationsAbility to customize messaging based on different audiencesAgile and creative problem-solving skills with an entrepreneurial mindsetSelf-motivated with the ability to work independently and manage a sales territoryStrong organizational skills with attention to detailRequirementsA valid driver's license and reliable transportation (registered and insured).Providence Care is an Equal Opportunity Employer.
