Licensed Care Manager - Home Health & Community Outreach

PruittHealth is seeking a Licensed Practical Nurse to serve as a Licensed Care Manager for Home First in Dekalb County. You will screen referrals, coordinate admission processes, and collaborate with providers to ensure appropriate services and medication reviews in patients’ homes. The role requires a valid LPN license, a driver’s license, CPR certification, and at least five years of nursing experience, with hospice or SOURCE care preferred.
