Job Overview We are seeking a highly skilled and dedicated Physical Therapist to join our team. The ideal candidate will possess extensive clinical expertise, including experience in various healthcare settings such as hospitals, long‑term care facilities, and outpatient clinics. As a Physical Therapist with UPLIFT HOMECARE, LLC, you will deliver exceptional patient care, coordinate treatment plans, and support our mission to enhance quality of life through personalized healthcare services. Responsibilities Provide skilled physical therapy in accordance with the physician’s plan of treatment. Assist the physician in the evaluation and assessment of the client’s level of functioning by applying appropriate tests. Assist in the development and revision of the plan of care. Treat clients to relieve pain, develop and restore function, and maintain maximum performance using appropriate physical therapy treatments, modalities, and procedures as stated in the plan of care. Observe, record, and report the client’s reaction to treatment and any changes in the client’s condition to the physician, Clinical Manager, and Case Manager. Educate client, caregiver, and health‑care team in the phases of physical therapy, use of prosthetic and orthotic devices, and durable medical equipment as appropriate. Provide staff in‑service education as directed by the Clinical Manager. Assess and regularly reevaluate the needs of the client; develop, implement, and revise the client’s plan of care. Initiate diagnostic, preventive, and rehabilitative procedures as appropriate to the client’s care and safety. Assist with the plan of care and maintain continuity of multidisciplinary clients whenever needed to ensure good coordination of care. Promote coordination of care through interdisciplinary case conferences and communication with physicians and field staff. Develop, prepare, and maintain individualized client care progress records with accuracy, timeliness, and in accordance with agency policy and procedures. Keep abreast of physical therapy trends and knowledge for service provision, documentation, and care coordination. Qualifications Current Physical Therapy license in the State of Florida. Two years of experience as a Physical Therapist working with diverse populations, preferably including pediatrics, geriatrics, individuals with developmental disabilities, or those requiring hospice & palliative medicine. Level 2 AHCA certification. Valid driver’s license, auto insurance, and reliable transportation. Current CPR certification. One year of OASIS experience. #J-18808-Ljbffr
