Bilingual Community Outreach Specialist
Upward Health, Inc.
Upward Health, Inc. is seeking a Community Outreach Specialist to educate communities about our in-home care programs and enroll patients in our services. The COS will manage a personal caseload, primarily via phone outreach, with in-person visits as needed, and will work under the Outreach Manager to ensure successful patient engagement.
