Job Summary:

Responsibilities:

• 
  Evaluate patients functional status and develop individualized treatment plans.
  
  
• 
  Deliver evidence-based occupational therapy interventions to promote recovery and independence.
  
  
• 
  Document patient progress and treatment outcomes accurately within required timeframes.
  
  
• 
  Collaborate with the healthcare team to coordinate care and adjust treatment plans as needed.
  
  
• 
  Ensure compliance with all safety, privacy, and regulatory standards during home visits.
  
  
• 
  Maintain reliable transportation and complete patient visits as scheduled.
  
  

Qualifications:

• 
  Hold an active state occupational therapy license.
  
  
• 
  Have a minimum of 1 year recent adult home health experience.
  
  
• 
  Possess current CPR and BLS certifications.
  
  
• 
  Proficient in using EMR systems such as HomeCare HomeBase.
  
  
• 
  Demonstrate excellent communication, time management, and organizational skills.
