Job Title Contingent Clinical Care LVN/LPN PT - MC Address South Elgin, IL Job Description Administer oral, injectable, intravenous, and topical medications. Evaluate complex patient conditions, monitor pain levels, and document treatment responses. Perform routine assessments to monitor patient health and ensure overall well-being. Manage patient access to healthcare and connect individuals with community resources for medication administration, self-care techniques, and disease management. Accurately document patient medical history using electronic health records. Keep an organized, well-stocked, clean clinic at all times. Critical Information Work schedules will be created in 6-week blocks following the 3 weeks of training/orientation; typical shifts will be between 7-10 hours during clinic operating hours. This is an onsite position; attendance is required for patient-facing duties. Education/Licenses Needed The LPN/LVN must have a full, active, and unrestricted license as a Licensed Practical or Vocational Nurse in a State, Territory or Commonwealth of the United States or District of Columbia. Minimum of one year of related work experience in a healthcare setting. Benefits Benefits are available to full-time employees after 90 days of employment.
