LPN - Home Health

Home Health LVN

This is a 13-week contract position.

Monday to Friday, day shifts with 5 × 8 hour shifts per week.

We are seeking a dedicated and experienced Licensed Vocational Nurse (LVN) for home health care. The ideal candidate will provide routine patient visits and ensure patients receive the highest quality of care.

Responsibilities:

• Conduct routine home health visits as required.
• Complete necessary documentation and maintain patient records.
• Collaborate with healthcare team members to provide comprehensive patient care.
• Adhere to care plan and update it as necessary based on patient progress.
• Ensure compliance with health and safety regulations.

Required Experience / Certifications / Licensure:

• Current LVN license.
• Minimum of 32 patient points required per week.
• Recent home health experience is required.
• HomeCare HomeBase (HCHB) experience preferred, but may consider candidates willing to train at a reduced rate for the first two weeks.
