At Northwestern Medicine Palos Hospital, we believe that a smooth transition from the ER to inpatient care is crucial for optimal patient outcomes. Our ER Transitional Care department is dedicated to ensuring that patients receive the highest quality of care, with a focus on safety, comfort, and a seamless transition to the next phase of their medical journey. In this role, you will care for patients who are boarding in the Emergency Department while awaiting an inpatient bed. You will deliver comprehensive nursing care through patient monitoring, assessments, medication and infusion management, and other essential clinical interventions. Opportunities to collaborate with inpatient teams, including supporting discharge processes, are also part of this position. We are seeking nurses with a strong background in Telemetry or Med Surg for this position.

The Staff Nurse reflects the mission, vision, and values of NM, adheres to the organization's Code of Ethics and Corporate Compliance Program, and complies with all relevant policies, procedures, guidelines and all other regulatory and accreditation standards.

Responsibilities include assessing and recording patient status and identifying patient care needs upon admission/point of care entry, organizing assessment health data, identifying problems that need immediate attention, validating diagnoses with patient/family and other health care team members, documenting diagnoses, initiating Problem List at the time of admission, incorporating principles of age-specific care into the patient's plan of care, documenting that the patient's individualized plan of care has been discussed with and agreed upon by the patient/family, beginning plans for discharge at time of admission, identifying patient teaching needs on admission and throughout hospital stay, arranging conferences with patient and/or family and other members of health team when indicated to plan care, prioritizing and organizing nursing interventions that are evidence based and reflective of established standards of care, providing individualized, mutually agreeable, patient and family education that is founded on evidence-based practice and research guidelines, documenting all assessments, interventions and patient responses, reviewing consistently the patient's health data to keep the Plan of Care updated, assuring that revisions to the plan of care are systematic and ongoing and are driven by measurable realistic goals, documenting the patient's progression toward identified outcomes, identifying unmet outcomes and providing direction for continuity of care, reassessing nursing actions for effectiveness in achieving desired outcomes, communicating with patient/family and health care team members re: patient care and nursing's role in the provision of care, collaborating with patient/family and healthcare team members re: the patient's progress/plan of care, presenting relevant patient information at multidisciplinary meetings/conferences for feedback from peers, and making appropriate referrals as needed for continuity of care.

Qualifications

Required: Bachelor of Science in Nursing (BSN), Current State of Illinois Registered Professional Nurse, American Heart Association Basic Life Support (BLS), Strong interpersonal skills, Ability to collaborate with others, Strong analytic abilities, Associate Degree Nursing (ADN) acceptable with commitment to obtain a BSN from an accredited program within three years of first day of employment in clinical nurse role.

Preferred: Experience in specific clinical area.
