The Opportunity

This home health PT role pays $101,000-$112,000 and comes with relocation assistance for candidates moving to the Denver area. You'll work one-on-one with patients in their own homes, building the kind of continuity and rapport that's harder to find in a clinic schedule stacked with back-to-back visits.

What Your Week Looks Like

• Perform comprehensive evaluations and review medical histories to assess each patient's condition
• Build individualized treatment plans around each patient's rehabilitation goals
• Guide patients through therapeutic exercises to restore mobility and function
• Educate patients and families on injury prevention and the recovery process
• Track and document progress, adjusting plans as patients advance
• Coordinate with a multidisciplinary team to keep care aligned across providers
• Maintain accurate, compliant documentation throughout

What We're Looking For

• Master's degree in physical therapy from a CAPTE-accredited program
• Active PT license in Colorado (or eligible for licensure)
• Current CPR certification
• Valid driver's license, with reliable transportation to patient homes across the service area
• Minimum nine months of experience as a physical therapist
• Community or home health experience preferred
