Medical Social Worker II - Grade 10

Job Summary The primary role of the Medical Social Worker II is to assist patients and families/caregivers to cope with social, emotional, and practical issues related to the patient’s illness. Under the general direction of the Social Work Manager/LCSW, you will deliver age‑appropriate social work care in accordance with agency policy, procedure, state, and federal regulations. This role is an integral member of the healthcare team, providing assessments, coordination, treatment planning, information, and referral to community resources to meet complex patient and family needs in hospital and clinic settings. Essential Responsibilities Provide psychodynamic interventions, crisis intervention, grief/bereavement counseling, problem solving, stress reduction, and develop healthy coping strategies in individual, family, and group settings. Offer counseling for disease acceptance and understanding. Develop and implement individualized Plans of Treatment to help patients and families address social, emotional, financial, and environmental factors impacted by illness. Complete psychosocial assessments and partner with patients to identify needs and create mutually agreeable treatment plans. Discuss care options proactively, including Kaiser resources and external community/government resources for short- and long-term care planning. Collaborate with other disciplines to assess, plan, and provide services using biopsychosocial information. Advocate for patients to obtain appropriate services within Kaiser and the community. Assist patients and families with care planning and discharge planning. Review, evaluate, and prioritize written and oral referrals. Maintain documentation, records, and data collections. Complete required documents promptly and accurately. Serve as part of the Skilled Nursing Facility Team to ensure timely placement of Kaiser members in nursing facilities. Liaise between patients and Kaiser, maintaining positive relationships and continuity of care. Identify appropriate levels of care and facilities for referred patients, and obtain placements when applicable. Collaborate with internal and external resources to meet mutually agreed objectives. Provide information and referral to community resources as requested. Coordinate information exchange between Kaiser, families, members, and skilled nursing facilities. Determine application of Kaiser, Medicare, and Medi-Cal benefits to specific patient situations. Participate in Utilization Management and Quality Assurance activities. Coordinate communication between regional offices, clinics, hospitals, and field staff, triaging phone calls from members and families. Work with referral sources to clarify and complete required clinical and psychosocial information. Perform other related duties as necessary. Basic Qualifications Step I: 0-2 years of social work experience. Step II: 2-4 years of social work experience within the last five (5) years. Step III: 4+ years of social work experience within the last ten (10) years. Education Master’s in Social Work accredited by the Council on Social Work Education. License, Certification, Registration N/A Additional Requirements Demonstrated ability to work on a multidisciplinary team. Solid psychosocial assessment skills. Knowledge of chronic and acute disease impacts on patient and family functioning. Excellent oral, telephone, and written communication skills. Computer-literate, preferably experienced with automated clinical information systems. Ability to handle a demanding workload with multiple tasks efficiently. Independent yet collaborative team member. Strong understanding of benefits and coordination of care details. Willingness to work in a Labor Management Partnership environment. Refer to collective bargaining agreement for position specifications. Preferred Qualifications LCSW preferred. At least one year post‑MSW experience in a health care setting; MSW internship considered. Job Details Primary Location: California, South San Francisco, S. San Francisco Hospital. Weekly Hours: 1 (call‑in/on‑call) Shift: Day (8:00 am‑4:30 pm or 2:30 pm‑11:00 pm) Workdays: Sunday–Saturday Working Hours Start: 08:00 AM Working Hours End: 04:30 PM Travel: None Work Setting: Onsite Pay Range: $60.58 – $71.37 per hour Kaiser Permanente strives to offer a market‑competitive total rewards package and is committed to pay equity and transparency.
