Program/Project Manager

Contractor must:


1. Regularly communicate and coordinate with ACBH staff.


2. Organize and attend project team meetings in person and online as needed, including preparing meeting agendas, presentation materials, and meeting minutes.


3. Prepare a project schedule and update it monthly.


4. Support the development and implementation of a strategy for Pre-Release Services Readiness Assessment of CalAIM Justice Involved Initiative through meetings with County staff and reports.


5. Support the development and implementation of the CalAIM Justice Involved Initiative infrastructure and processes for providing 90-Days Pre-Release Medi-Cal covered services for incarcerated population by October 1, 2024.


6. Support the provision of Pre-Release Services and Behavioral Health Care Linkages for incarcerated eligible individuals.


7. Evaluate existing ACBH Care resources, processes, and infrastructure and provide recommendations on improvements.


8. Collaborate with internal and external strategic and operational partners and support and/or facilitate communication among different stakeholders.


9. Engage in ACBH activities to operationalize program milestones for each CalAIM goal in compliance with DHCS requirements.


10. Verify any CalAIM policy changes and communicate them to ACBH leadership.


11. Monitor and report on the implementation status of the CalAIM related initiatives.


12. Monitor and report on all milestones from DHCS to the ACBH CalAIM Executive Sponsor, Chief Nursing Officer.


13. Apply knowledge and experience in Change Management.


14. Provide Project Scoping, Training to County staff working on the CalAIM Justice Involved Initiative and other stakeholders, and Project Implementation.


15. Submit required reports in a timely and professional manner.


16. Document potential barriers to a successful implementation of Justice Involved CalAIM Initiative and develop a suite of solutions to overcome those barriers.


17. Provide subject matter expertise and project management regarding the development and implementation of CalAIM-related policies and procedures.
18. Not charge for travel expense and will not be allowed. The cost quoted must include all taxes (excluding sales and use tax) and all other charges, including travel expenses
