About This Role As an RN Hospice PRN at Cleveland Clinic, you will provide compassionate care to patients in their homes, ensuring comfort and preventing readmissions. Highlights Pay: $41.00/hour Location: Independence, OH Facility: Cleveland Clinic Fairview Hospital Schedule: Per Diem, Night shift What You'll Do Provide direct nursing care in accordance with established healthcare organization policies and procedures. Implement and monitor patient care plans. Provide supervision to LPNs, Home Care Aides, and families/caregivers involved in patient care. Perform accurate and complete admission assessments and reassessments within a specified time frame. Assess patients for safety risks, abuse, and neglect. Serve as a primary coordinator of all disciplines for well-coordinated patient care. Assess and document patients' pain using appropriate pain scale and manage pain. Prioritize and document patient problems based on assessed needs in collaboration with patients, family, and healthcare team. Maintain complete and up-to-date documentation regarding patient condition and care effectiveness. Escalate changes in patient condition to the patient’s physician as appropriate.
