Job Details:

• Location: 275 Hospital Drive, Ukiah, California, 95482, United States
• Specialty: Physical Therapist Outpatient
• Schedule: 8-hour shifts, Monday through Friday, with a variable start time between 7:30 AM and 8:00 AM; variable lunch break of 30 or 60 minutes; no weekends required
• Required Qualifications: Doctor of Physical Therapy (DPT) degree, licensed as a Physical Therapist in the state of practice, and CPR or Basic Life Support (BLS, HS-BLS, RQI BLS) certification
• Preferred Qualifications: Two years of outpatient physical therapy experience
• Additional Responsibilities: Completing evaluations, establishing individualized treatment plans, providing therapy interventions, documenting clinical activities, supervising assigned assistants, and performing related duties as assigned
• Vaccinations: All required vaccinations as a condition of employment and annually thereafter, where applicable
