Responsibilities:

• Assist with transporting residents to and from medical appointments, therapy sessions, and social activities
• Provide emotional support and companionship to residents during transportation
• Monitor resident vital signs and report any changes to nursing staff
• Assist with wheelchair and stretcher transfers
• Maintain accurate records of transportation activities and resident vital signs
• Collaborate with nursing staff to develop and implement individualized care plans
• Participate in orientation and training programs to ensure compliance with facility policies and procedures

Requirements:

• Current CNA certification
• High school diploma or equivalent required
• Ability to lift, transfer, and move residents as needed
• Strong communication and interpersonal skills
• Ability to work effectively in a fast-paced environment
• Valid driver's license
