North Starr Post Acute Care - Summary: The Physical Therapist will be responsible for evaluating, diagnosing, and developing individualized treatment plans for patients with injuries, disabilities, or other health conditions that limit their ability to move or function. As the PT you will develop personalized care plans to help patients manage pain, restore mobility, prevent disability, and improve overall quality of life. Hiring for Full Time and PRN. Essential Duties and Responsibilities: · Evaluate and assess patients' physical conditions, medical histories, and test results to accurately diagnose movement dysfunction. · Develop customized, evidence-based treatment plans that include therapeutic exercises, manual therapy, and the use of specialized equipment to reduce pain, restore function, and prevent disability. · Administer and supervise therapeutic treatments, including manual exercises, massage, electrical stimulation, and ultrasound, modifying the plan as needed based on patient progress. · Educate patients and their families or caregivers on proper techniques for at-home exercises and preventative care to support long‑term recovery and well‑being. · Help patients with the proper use of assistive devices, such as crutches, canes, or braces, providing training and recommending adjustments as necessary. · Maintain accurate and timely patient records, documenting evaluations, treatment plans, progress, and discharge summaries in compliance with regulatory and organizational standards. · Collaborate effectively with physicians, surgeons, occupational therapists, and other healthcare professionals to provide comprehensive, coordinated patient care. · Provide clinical guidance and supervision to Physical Therapist Assistants and students, ensuring the highest standard of patient care is maintained. · Stay up-to-date on the latest physical therapy techniques and practices through continuing education and professional development. · Perform other duties as assigned by Supervisor and/or Manager The above listing of duties and responsibilities is not intended to be all-inclusive but rather to serve as a description of the range of duties and general nature of the position. Education and Work Experience: · BS or Master's Degree in Physical Therapy from a physical therapy program approved by the American Physical Therapy Association · Assist and direct treatments given by Physical Therapist Assistants. · Knowledge of responsible use of facility equipment, best practices for equipment use and preventative maintenance procedures. · Experience with recommending assistive equipment to patients and outpatient or home health follow‑up programs. · A strong sense of empathy and a compassionate outlook on PT. Certificates, Licenses, Registrations: · Current Physical Therapy license for state of operation. $65–$75 per hour Knowledge and Skills: 1. Excellent verbal and written communication skills are mandatory. 2. Must be detail oriented and comfortable working in fast‑paced and rapidly changing environment. 3. Teamwork collaboration is essential to the organization, so it is critical that the manager communicates and interacts well with other team members and is able to build positive relationships across all levels within the organization. 4. Must possess excellent organizational skills and be extremely flexible to meet customer demands, in addition to being able to work independently and prioritize multiple tasks in a changing environment with a high degree of accuracy. 5. Ability to follow oral and written instructions. 6. Ability to positively interact with personnel, customers, family members, visitors, government agencies/personnel and the general public. 7. Interacts with customers, families, visitors, center and the Company's subsidiary personnel; 8. Carries out other tasks as requested in situations where hands‑on intervention/participation may be required. 9. Provide annual verification of negative TB skin test. Essential Job Functions — Physical & Work Conditions - Physical demands: frequent standing and walking; occasional sitting. Regular reaching (including overhead), handling/fingering, feeling, talking/hearing, and seeing. Occasional stooping, crouching, climbing stairs, balancing, and twisting; kneeling and crawling are not typically required. - Patient care and equipment handling: ability to assist with mobility and handle clinic equipment of approximately 20 lbs (pulling, lifting, carrying, pushing). Assistive devices and team support are available per protocol. - Work environment: primarily an indoor clinical setting; occasional travel up to ~5%. - Environmental factors: typical clinic conditions; no routine exposure to extreme temperatures, wet/humid environments, fumes, poor ventilation, dusts, gases, unprotected heights, or moving machinery. Occasional operation of automotive equipment and use of foot controls may be required.
