Summary Provides psychosocial assessment, care planning, intervention and evaluation to individuals and families undergoing treatment for patients and provides appropriate documentation of plan of care as well as progress notes documenting the patient’s response to interventions. Utilizes individual or family treatment modalities and makes use of appropriate referrals for community agencies and support services. Responsibilities Prioritization of Patients: Collaborate and communicate with ED Care Team members to identify patients in need of psychosocial assessment/interventions and/or immediate discharge needs. Prioritize workload between crisis patients, routine intervention patients, and patients with non‑critical discharge needs. Document interventions, progress notes, and adhere to escalation processes. Discharge and Transitions Planning: Address discharge needs and discuss options with the ED staff, patient, family, and other care team members to ensure safe discharge. Collaborate with the care team to determine post‑service needs and locate community resources. Coordinate psychiatric or addiction services, provide resources for patients at risk of abuse, neglect, or domestic violence, and identify resources for homeless patients. Collaborate with Primary Care Physicians and the patient/family on the discharge plan and ensure understanding of provided resources. Crisis Intervention: Act as a patient advocate. Address psychosocial needs of patient and family, identify and address crisis situations such as trauma, violence, abuse, neglect, death, or sexual assault, as well as life changes and dependency circumstances. Provide counseling, support, and contact appropriate agencies according to established procedures. Collaborate with involved care team members to develop a plan for patients requiring crisis intervention. Address Frequent ED Utilization: Develop and maintain care plans for patients who frequently seek ED care. Track high‑frequency users, assess reasons for ED use, and communicate with patients/families about appropriate use. Provide information, education, and coordinate with community outreach agencies to prevent inappropriate future ED use and secure a PCP. Communication: Communicate with team members, physicians, and other care providers, both verbally and through documentation. Deliver concise reports on patient progress and respond to treatment. Notify supervisors and relevant parties of significant issues, conflicts, or incidents. Ensure confidentiality of patient and family information. Client Liaison: Serve as a patient/family liaison, advocate, and resource. Explain treatment purpose and nature prior to initiation to minimize anxiety and enhance cooperation. Document patient/family grievances and respond appropriately. Clinical Skills: Demonstrate knowledge suitable for age groups served, assessing, providing, and interpreting age‑specific data for pediatric, adolescent, adult, and geriatric patients. Communicate effectively with patients across developmental stages. Quality Improvement: Participate in quality assurance and improvement activities. Collect and analyze data, contribute to CQI initiatives, and identify positive patient outcomes related to CQI programs. Additional Duties: Perform other duties as assigned, including assistance with behavioral health housing placements, observation for behavioral health patients, and departmental Community Liaison Projects.
