You will lead care from evaluation through discharge with full clinical autonomy and control over scheduling, productivity, and patient outcomes. Qualifications:

• Graduate from a physical therapy curriculum approved by the American Physical Therapy Association, or the Committee on Allied Health Education and Accreditation of the American Medical Association, or the Council of Medical Education of the American Medical Association and American Physical Therapy Association.
• Currently licensed in the State of Colorado.
• Two years’ experience in home health, outpatient rehab or hospital is preferred.

Responsibilities:

• Provide comprehensive physical therapy services to patients in their homes, focusing on individualized care plans.
• Assess patient needs and develop treatment strategies to enhance mobility and overall function.
• Educate patients and caregivers on therapeutic exercises and safety measures.
• Collaborate with healthcare professionals to ensure continuity of care and effective discharge planning.
• Document patient progress and adjust treatment plans as necessary, adhering to medical terminology and documentation standards.
• Utilize knowledge of anatomy and physiology to inform treatment decisions and improve patient outcomes.
