Salary range $130,000 to $145,000 a year./ppThe Advanced Practice Nurse will be responsible for, but are not limited to:/polliConduct visits to patients in accordance with local, state, and federal regulations./liliCommunicate visit findings to designated members of the interdisciplinary team./liliFacilitate access to appropriate support models of care (i.e., rehabilitation, on-going aggressive treatment)./liliConsult with other members of the interdisciplinary team to contribute to the patients plan of care./liliPrepare and maintain accurate patient records, charts, and documents./liliAssist in development of clinical practice guidelines/standards in support of quality care as needed./liliEnsure collaboration with patients, caregivers, medical directors, and members of the interdisciplinary team./li/olpEducation:/ppMasters degree in Nursing (MSN) or Doctor of Nursing Practice (DNP) from an accredited program./ppProfessional Experience:/pulliMust have a Nurse Practitioners license in the state of operations./liliPMHNP certification./liliMust have 1+ years experience working within Family, General Practice, Geriatric care, or related field./liliExperience in a nursing home, hospital, assisted living, or acute rehab space is a plus but not required./liliCommitment to ongoing professional development and clinical excellence./liliMust have a valid drivers license and a reliable source of transportation./liliMust be willing to travel locally./li/ulh2About Us/h2pThe CareOne mission is to define excellence within the health care community.
