Cardiology Home Health Physician Assistant

CenterWell Home Health seeks a Physician Assistant, Cardiology to provide in-home cardiac-focused care to seniors and adults with chronic conditions. In this role, you’ll perform comprehensive assessments, manage cardiovascular treatment plans, and collaborate with cardiologists, nurses, and therapists to support recovery and independence at home. Responsibilities include ordering and interpreting tests, educating patients and families, documenting in the EHR, and contributing to quality initiatives within a compassionate, team-based, patient-first culture.
