Job Summary:
Physical Therapist (PT) specializing in Skilled Nursing Facility (SNF) settings in Oklahoma. This role involves evaluating, planning, and providing rehabilitation services to patients with physical functioning disorders and wound care needs. The position emphasizes evidence-based practice, accurate documentation, and collaborative care to ensure optimal patient outcomes.
Job Details:
- Schedule: Days shift, 8 hours per day, minimum 40 hours per week.
- Required Qualifications: Valid Oklahoma state license; 1 year of physical therapy experience; new graduates with Level II experience are accepted; experience in long-term care (LTC) or rehabilitation settings preferred.
- Experience in evaluating and rehabilitating patients with physical functioning disabilities, wound care, and therapeutic interventions.
- Competence in utilizing standardized assessments, administering therapeutic procedures, and documenting clinical progress.
- Ability to support and supervise physical therapist assistants, rehabilitation aides, and students.
- Adherence to the American Physical Therapy Association (APTA) practice acts, ethics, and professional standards.
- Willingness to participate in team meetings, training sessions, and continuing education activities.
- Effective communication and documentation skills, including preparing discharge summaries and maintaining patient records.
- Ability to obtain necessary durable medical equipment (DME) and supplies for patient care.
