About the Position

Job Title: RN Case Manager Profession: Registered Nurse Specialty: Hospice Duration: 13 weeks Shift: Day Hours per Shift: 8 Experience: Minimum 1 full year Adult Home Health Experience or 1 full year Hospice experience License: Must have license in hand Certifications: CPR, BLS Must-Have: Must have recent specialty experience in hospice Description: Provides, coordinates, and directs the provision of home nursing care according to physicians' orders based on agency policies and procedures through the competent application of the nursing process. Responsible to Clinical Manager. Directly supervises and evaluates Home Health Aides and Licensed Practical Nurses/Licensed Vocational Nurses. Home visits are required. Periodic office meetings will take place. Must have reliable transportation, current driver's license, and agency-required liability insurance. Average number of patients per day is 5-8. Each type of visit and activity (documentation, travel, etc.) holds a different point value. Contractors will be assigned between 5-10 patients per day, which equates to roughly 6-8 points. No guaranteed hours are available. If there is a situation where the contractor cannot meet their financial needs due to low patient census, please discuss floating the contractor to a nearby location to pick up hours. Types of cases include Start of Care, Wound Care, and Follow-up Visits. First-timers are not accepted; recent specialty experience is required. Weekend requirements vary per location and profession; please discuss during the interview. On-call requirements vary; please discuss during the interview. Must be willing to travel up to 75 miles from the home clinic. Mileage is reimbursed at the IRS standard rate. Compensation is provided from patient home to patient home. The commute to and from the first and last patient is not billable. Local candidates are accepted at the same rate. No pending licenses are accepted; must have license in hand. All professionals must be proficient with HomeCare HomeBase and must have used it within the last 6 months. If not, they may apply but will be required to take a retraining course during compliance. RNs, PTs, and OTs are required to be proficient in OASIS. Travelers must confirm awareness that this is a home health assignment and they will be responsible for driving to patients' homes daily and to the clinic for weekly meetings. Travelers must have a vehicle, driver's license, and auto insurance. The mileage radius for this job is 50-75 miles, which may result in longer drives between patients. Travelers must be comfortable with that. Travelers must understand that expectations are to accept every patient on their schedule regardless of how far they are from their previous appointment. Declining patients may result in a cancel. Travelers must understand that all documentation must be completed within 24 hours of the appointment with the patient. Delayed and incomplete documentation that impacts billing and follow-up patient care will also result in a cancel. Travelers must recognize that there are no guaranteed hours with this contract. Clinicians must hit weekly productivity points of 30, equating to 40 hours per week. All overtime must be pre-approved. If clinicians are not receiving enough patients each week to support their financial needs, they will have the opportunity to float to another nearby location to pick up patients.
