Case Manager Description
-Lead the Care Coordination process within an assigned territory, balancing patient and physician needs with program requirements, and m

-Assess physicians' needs and develop proactive action plans to prevent therapy delays, coordinating patient-related information with all relevant stakeholders.
-Stay updated on reimbursement processes, insurance plans, and financial assistance programs, assisting with insurance approvals/denials, copay -assistance, and prescription management.
-Provide education to patients and healthcare providers on insurance requirements, therapy initiation, and relevant disease/product information.
-Exhibit leadership by driving action plan execution, supporting special projects, recommending process improvements, and sharing knowledge with team members.

Case Manager Requirements

1+ years of previous case manager or relevant healthcare experience


High School Diploma or equivalent
