POSITION PURPOSE

 

This position is located within the Behavioral Health Department at Haak’u Health Center (HH), Acomita, New Mexico. The Behavioral Health Department consists of an outpatient mental health clinic.

 

ESSENTIAL DUTIES, FUNCTIONS & RESPONSIBILITIES

 

This list of duties and responsibilities is illustrative only of the tasks performed by this position and is not all-inclusive.

 

·         Deliver evidence-based psychotherapy to children, adolescents, and adults experiencing mental health issues in individual, family, and group formats.

·         Maintain communication with community providers, clinics, and schools to support patient care.

·         Deliver in-services, as appropriate, for community-based partners (e.g., tribal programs, Acoma Behavioral Health).

·         Conduct clinical assessments for diagnosis, case conceptualization, and treatment planning/delivery/evaluation.

·         Document all patient contact (i.e., initial evaluations, progress notes, and patient contacts) in EHR.

·         Provide support for crisis services—refer patients to the closest emergency room, communicate with ambulance and hospital staff as needed

·         Conduct risk assessments to ascertain whether patients might be in imminent danger (e.g., suicide, homicide, alterations to mental status).

·         Develop community-based program initiatives to support the mental health and functioning of communities served.

·         Review client rights and responsibilities forms with patients and ensure their understanding of them.

·         Participate in treatment team meetings with providers and evaluate patient progress and need for additional services. 

·         Provide referrals to support services as needed—nutrition services, Social Security disability, medical, legal advocacy, etc.

·         In collaboration with clinic providers, participate in performance improvement projects—assemble statistics and present to staff.

·         Provide basic education to patients about medication management, accessing the pharmacy and communicating with their providers.

·         Protect pt. confidentiality and adherence to HIPAA.

 

MINIMUM MANDATORY QUALIFICATIONS:

 

·         Experience: At least three years of experience delivering psychotherapy in a community-based setting.

 

·         Education: Master's Degree in Counseling or Social Work (i.e., LMHC, LPCC, LMSW, LCSW)

 

·         Must pass health and background screenings.

 

 

MANDATORY KNOWLEDGE, SKILLS, ABILITIES & OTHER QUALIFICATIONS:

 

·         Strong interpersonal communication skills by phone and in writing.

·         Knowledge of behavioral health disorders and interventions, experience with screening for behavioral health disorders, including substance abuse.

·         Experience with psychopharmacology for behavioral health conditions and basic knowledge of commonly used medications in behavioral health

·         Demonstrated ability to collaborate and communicate effectively in a team setting.

·         Ability to maintain professional relationships with patients

 

POSITION PREFERENCES

 

·         Bilingual skills in English and the Keres native language.

·         Prior experience working with Indian Health Services (IHS), a tribe or tribal organization.

 

WORK ENVIRONMENT:

 

Work is performed in an office setting or outdoor work environment with a moderate noise level. The work environment may involve exposure to physical health risks, such as bloodborne pathogens or working with chemicals. Evening and/or weekend work may be required. Extended hours and irregular shifts may be required.

 

PHYSICAL DEMANDS: The physical demands described here are representative of those that must be met by an employee to successfully perform the primary functions of this job. While performing the duties of this job, the employee may be required to frequently stand, walk, sit, bend, twist, talk, and hear. There may be prolonged periods of sitting, keyboarding, reading, as well as driving or riding in transport vehicles. The employee must occasionally lift and/or move up to 50 pounds. Specific vision abilities required by this job include reading, distance, computer, and color vision. Talking and hearing are essential to communicate with patients, vendors, and staff.

 

MENTAL DEMANDS: There are many deadlines associated with this position. The employee must also multi-task and interact with a wider variety of people on various and, at times, complicated issues.

All employees must uphold all principles of confidentiality and patient care to the fullest extent and as required by Federal law and HH policies and procedures. This position has access to sensitive information and a breach of these principles will be grounds for immediate termination.

The incumbent will require a Tier 2 Federal Background Investigation for Federal employment, as determined by fingerprint check results and appropriate background investigation. An incumbent found not suitable after hire will be removed from this position.

Incumbent may be required to operate a Government-owned or leased motor vehicle for official purposes and requires a state operator's license (Class D).

Measles and Rubella immunization is required for selectees born after 1957. Seasonal Influenza immunization is required for civilian health care facility staff working in Indian Health Service health care facilities.
