RN Case Manager – Home HealthLocation: Albany, OR and surrounding areasWage $50.00 to $55.00 depending on experienceHours: M-F, 8am-5pm, full-timeSignature Albany Home Health is seeking a skilled and compassionate Registered Nurse (RN) Case Manager to provide high-quality home health care to patients in the comfort of their homes.This full-time role is ideal for an RN with experience in home health, care coordination, and patient-centered care, who thrives in an independent, field-based environment while collaborating with an interdisciplinary team.
