Social Worker

Responsibilities Provide assigned patients with activities, group sessions to plan objectives, stimulate behavioral change and to process effectively. Provide evaluations and psychosocial assessments for patients in collaboration with the td team. Initiate, develop, coordinate and evaluate treatment plans in providing appropriate treatment for individual patients. Prepare and keep a current case record for each assigned patient according to HealthAlliance Hospital, OASAS, and JC standards. Participate as a team member in conferences, treatment planning on behalf of patient. Attend in-service training and keep professionally updated through reading and attending seminars. Provide lectures, workshops/training for patients and staff as assigned. Strictly adhere to all confidentiality requirements imposed by law and professional conduct. Observe adherence to current policies and act as a resource to the director for program recommendations. Perform any other duties as assigned. Qualifications / Requirements Experience Minimum one (1) year experience providing mental health treatment preferred. Experience using one or more evidence‑based treatment approaches preferred. Experience working with individuals with SUD preferred. Education Masters of Social Work or Mental Health Counseling from an accredited program. Licenses / Certifications Current NYS Masters Level licensed QHP or licensure eligible with limited permit. #J-18808-Ljbffr
