Responsibilities
- Conduct comprehensive PT evaluations and develop individualized plans of care for residents.
- Deliver evidence-based PT interventions including therapeutic exercises, gait training, balance training, and ADL rehabilitation.
- Monitor progress, document outcomes, and adjust treatment plans to maximize function and safety.
- Collaborate with the interdisciplinary team, communicate with families, and contribute to discharge planning.
- Maintain accurate documentation in the electronic health record and ensure compliance with regulatory standards.
- Promote fall prevention, infection control, and safe patient handling during sessions.

Requirements
- Active physical therapy license as required by state regulations.
- Ability to work in a skilled nursing facility setting; travel assignment experience is a plus.
- Experience in SNF or geriatric rehabilitation is preferred.
- Strong clinical assessment, documentation, and communication skills; proficient with standard EHR systems.
