Physical Therapist - Travel Contract

Minimum Requirements

• Bachelor's, Master's or Doctoral Degree in Physical Therapy from accredited schools approved by the APTA
• Active State License is Required to Start the Assignment
• BLS Certification May Be Required from AHA or ARC
• 1+ year of PT experience required.
• No new graduates.
• AZ Physical Therapist license required to start.
• Physical Therapist checklist required to start.
• Competency exam required to start.
• Hep B required to start.
• TB required to start.
• Flu required to start.
• Tdap required to start.

Assignment Details

• 13-week assignment.
• Start 2+ days per week at 7am and end 2+ days per week at 7pm. Last patient is taken at 6pm.
• Clinic hours are Monday 10am-7pm, Tuesday 7am-4pm, Wednesday 10am-7pm, Thursday 7am-4pm, and Friday 7am-4pm.
• Floating may be required to another clinic approximately 15 minutes away.

Location Highlights

Surprise, Arizona offers access to both metro Phoenix conveniences and Sonoran Desert recreation. Surprise is about 20 miles from Glendale's Westgate Entertainment District and State Farm Stadium, giving travelers access to shopping, dining, and major events within a short drive. White Tank Mountain Regional Park is roughly 15 miles away and features hiking, mountain biking, and desert wildlife viewing across more than 29,000 acres. The city also includes Surprise Stadium, known for Major League Baseball spring training, and has convenient roadway access via Loop 303 for commuting across the northwest Valley.
