What You'll Do

• Conduct screenings, evaluations, and develop personalized care plans for patients with physical function disorders or wound care needs.
• Administer therapeutic interventions including manual techniques, ambulation, exercises, modalities, and assistive devices.
• Monitor patient progress, adjusting treatments based on clinical assessments and regulatory standards.
• Document services, treatment outcomes, and discharge summaries accurately in electronic health records.
• Collaborate with healthcare team members to ensure coordinated patient care.
• Support and supervise physical therapist assistants, rehab aides, and students.
• Participate in meetings, trainings, and professional development activities.
• Obtain necessary DME and supplies as per clinical guidelines.
• Adhere to professional practice acts, ethical standards, and facility policies.

What You Need

Required:

• Valid Massachusetts Physical Therapy license
• CPR certification
• 2+ years experience in long-term care or rehabilitation
• Strong communication and clinical skills

Preferred:

• Experience supervising support staff
• Knowledge of regulatory and documentation standards
