MetroPlusHealth in New York is seeking a Nurse Care Manager to review and approve Plans of Care, CANS-NY assessments, and UM reviews for HCBS services. The CM coordinates with Health Home teams to ensure appropriate access and level of care for medically fragile children. This is a full-time, on-site role with standard business hours.
