Working under the direction of a Unit Manager, you will be responsible for the assessment, planning, and implementation of individualized resident care plans. Key Responsibilities

• Participate in the comprehensive assessment of resident health status and needs.
• Develop and contribute to individualized care plans for residents, ensuring alignment with physician orders and facility protocols.
• Implement nursing interventions and provide direct resident care, including medication administration, wound care, and other treatments as prescribed.
• Monitor and evaluate resident responses to care, documenting observations and making adjustments to care plans as necessary.
• Collaborate effectively with the interdisciplinary team, including the Unit Manager, to ensure continuity and quality of care.

Required Qualifications

• Graduation from an accredited Registered Nurse (RN) Program or Licensed Practical Nursing (LPN) Program.
