As our Social Worker, you will be a compassionate advocate, providing vital support and guidance to individuals and families facing complex challenges. Every day, you will conduct psychosocial assessments, develop care plans, provide counseling and crisis intervention, and connect clients to resources. You'll advocate for clients and collaborate with multidisciplinary teams to ensure holistic support and promote well-being. The Social Worker is responsible for performing social work assessments and interventions for inpatient, outpatient, and emergency department patients. Dedicated to patient and family centered care, valuing self-determination, and skilled at managing multiple priorities while engaging in compassionate ethical problem-solving. As a member of a multidisciplinary health team, perform comprehensive assessments and develop treatment plans that integrate medical, social, and resource issues. Work collaboratively with all healthcare team members to implement treatment plans that support patient-centered care. Functions include crisis intervention, patient/family intervention, high-risk screening, brief counseling, referral for financial or other resource needs, and facilitation of family/patient meetings. Assist in post-acute placement on complex discharges and engage appropriate agencies or community resources when high-risk patients are identified. Establish and maintain professional relationships with physicians, case managers, nursing staff, county agencies, community resources, patients, and families. Serve on hospital committees and present in-services to hospital staff. Required At least two years of clinical and/or medical social work experience in a medical facility. Master’s Degree from a School of Social Work accredited by the CSWE. Knowledge of contemporary behavioral and systems theories relevant to health care; end-of-life dynamics; grief and bereavement counseling; substance abuse interventions; support for victims of abuse, neglect, or violence; community or financial resources for underserved or vulnerable populations. Understanding of SSDI, SDI, and other governmental/county requirements and programs. Comprehensive knowledge of OBRA and Title 22. Excellent customer service, presentation, communication, and analytical skills. Ability to manage multiple tasks, work independently, and maintain detail orientation. Licensed Clinical Social Worker (LCSW). Preferred One year of clinical and/or medical social work experience in an acute psychiatric facility.
