Physical Therapist

Physical TherapistCentral Health Management, Inc. is seeking a full time Physical Therapist for a Monday - Friday 8 hour shifts providing care for patients in their home in Southwest Virginia.Provide physical therapy services to patients under appropriate guidelines.· Implement and conduct treatment as outlined in treatment plan.· Treats clients to relieve pain, develop and restore function and maintain maximum performance using appropriate physical therapy treatments and procedures within the plan of care.· Monitor patient's response to treatment and modify treatment as indicated in accordance with physician's orders.· Interpret and communicate evaluation findings to patients, family and other staff members.Qualifications:Active and Valid Physical Therapy Licensed in VirginiaValid Driver's licenseBLS/CPR certifiedPrefer Home Health experience, not requiredEqual Employment OpportunityCentral Health Management Inc. encourages all applicants meeting the qualifications of the role to apply regardless of national origin, age, gender, gender identity, race, religion, disability or veteran status.
