As the Thrombosis RNCC you will support comprehensive anticoagulation and thrombosis care coordination across the lifespan, with a focus on patient safety, follow-up, education, surgery coordination, and quality initiatives. This role works collaboratively with providers, pharmacy, PSC, and multidisciplinary teams to improve continuity of care and ensure timely patient management.
The opportunity includes:
- Coordinate care for patients receiving anticoagulation and thrombosis-related services across inpatient and outpatient settings.
- Monitor and assist with follow-up for overdue INR testing, including outreach through phone calls, portal messaging, and coordination with providers/pharmacy teams as appropriate.
- Assist in identifying and addressing overdue office visits for anticoagulation patients, including collaboration with PSC, providers, and use of dashboards or reporting tools (e.g., Power BI) to support follow-up workflows.
- Support perioperative anticoagulation planning by gathering initial surgical information from patients and/or surgical offices, including completion of standardized intake forms and documentation.
- Coordinate mailing or distribution of surgery plans, educational materials, and medication samples as needed.
- Review hospitalization and emergency department notifications related to thrombosis or anticoagulation care and provide appropriate follow-up, including patient outreach, coordination of care, and re-education when indicated.
- Track and document anticoagulation-related events using standardized encounter workflows and templates, including review of outside records and CareWeb documentation.
- Provide anticoagulation education and reinforcement to patients and families as needed, including medication compliance, safety precautions, and perioperative instructions.
- Assist with transition of care coordination for thrombosis and hematology patients, including facilitation of referrals and ensuring successful establishment with outside providers when needed.
- Respond to patient and provider questions related to existing surgery plans, anticoagulation instructions, or plan clarification.
- Support follow-up and monitoring related to the "7-2-1" rule and other thrombosis-specific clinical initiatives for patients starting anticoagulation therapy.
- Participate in ongoing review and maintenance of thrombosis and anticoagulation program documents, policies, workflows, and educational resources to support Anticoagulation Center of Excellence (AC COE) standards.
- Collaborate with pharmacy, nursing, providers, and administrative teams to improve workflows, identify care gaps, and support quality improvement initiatives.
- Utilize critical thinking skills to identify patients needing additional support, intervention, or escalation, rather than solely performing task-based follow-up.
- Maintain accurate, timely documentation within the electronic medical record and applicable tracking systems.
Knowledge:
- Comprehensive knowledge of nursing processes, healthcare systems, organizational structures, and community-based resources.
- Understanding of healthcare technology, medical equipment, and clinical supplies utilized in patient care delivery.
- Knowledge of state nursing regulations, Nurse Practice Act requirements, and applicable clinic policies and procedures.
- Knowledge of medical practice telephone triage protocols, nursing assessment techniques, and clinical decision-making processes.
- Understanding of medical record documentation standards, regulatory compliance requirements, and federal and state laws governing the confidentiality and release of health information.
Skills:
- Strong time management, problem-solving, multitasking, prioritization, and care coordination skills in a fast-paced clinical environment.
- Proficiency in utilizing electronic medical records (EMR/EHR) systems to review patient histories, document care, and support clinical decision-making.
- Excellent interpersonal and communication skills, with the ability to effectively manage sensitive, complex, and confidential situations.
- Skilled in assessing and triaging patients both in person and via telephone to ensure timely and appropriate care.
Abilities:
- Ability to evaluate patient needs, analyze treatment and care options, educate patients and families, and facilitate referrals to appropriate providers and community resources.
- Ability to engage patients and families in the development, implementation, and management of individualized care plans.
- Ability to accurately read, interpret, and implement physicians' orders, treatment plans, and documentation from interdisciplinary healthcare providers.
- Ability to accurately calculate medication dosages, administer injections, and monitor and document patient outcomes.
