Home Health Physical Therapist

Plan and administer prescribed physical therapy treatment for patients in their homes Assess and document patients' physical strengths to support physician evaluation and guide treatment goals Develop and implement rehabilitation programs consistent with physicians' Plans of Treatment and patient-team goals Provide PTAs and Home Health Aides with written care instructions Oversee PTA and Home Health Aide performance and documentation Document patient observations, interventions, and evaluations promptly and thoroughly Report patient progress to physicians, Clinical Manager, care team, patients, and families Submit required evaluations, plans, progress notes, and discharge summaries Help patients regain strength, mobility, and independence through personalized home healthcare Requirements Current and unrestricted Physical Therapy license Current CPR certification Experience collaborating with a team of healthcare professionals Valid state driver's license Personal vehicle liability insurance in accordance with residing state minimum required limits or specified higher limits One year of experience as a Physical Therapist (preferred) Core Competencies Demonstrates expertise in planning and administering physical therapy treatments, developing rehabilitation programs, and collaborating with healthcare teams to enhance patient outcomes. Holds a current Physical Therapy license and CPR certification, ensuring compliance with healthcare standards.
