Job Summary Physical Therapist – Home Health – Marysville, CA. Salary: $3,240 in 40 hours. Employment type: Full-time, Part-time, Contract. Location: In-Office, Marysville, CA, USA. Responsibilities Assess patients, develop and implement home-based PT plans, and monitor progress. Provide hands‑on therapy sessions in patients' homes to improve mobility and function. Set measurable targets and track patient progress within defined treatment pipelines. Document patient evaluations, progress notes, and treatment outcomes accurately. Coordinate care with physicians, nurses, and caregivers to optimize outcomes. Adhere to clinical guidelines, safety standards, and regulatory requirements. Demonstrate a growth mindset by pursuing ongoing professional development and certifications. Utilize scheduling pipelines to optimize visit efficiency and meet coverage targets. Educate patients and families on exercises, devices, and home safety to support independence. Collaborate with the team to share best practices and improve service delivery. Maintain equipment inventory and ensure proper sanitation of therapy tools. Advocate for patients to ensure timely access to needed therapies and services. Experience & Qualifications Licensed Physical Therapist with active CA license. Experience delivering home-based physical therapy and home safety education. Ability to develop, implement, and monitor PT plans with measurable targets. Proficient documentation of evaluations, progress notes, and outcomes. Strong collaboration with physicians, nurses, and caregivers to coordinate care. Knowledge of clinical guidelines, safety standards, and regulatory compliance. Experience using scheduling/pipeline tools to optimize visit efficiency. Demonstrated commitment to professional development and certifications. Strong patient and family education skills on exercises, devices, and home safety. Maintains equipment inventory and ensures proper sanitation of therapy tools.
