Physical Therapist

Physical Therapist (PT) Part-Time & Per Diem Pay: $65–$70 per hour Location: Citrus Heights, CA At Citrus Heights Post Acute, we're passionate about helping residents recover, regain strength, and return to the highest level of function possible. We are seeking a dedicated Physical Therapist to join our rehabilitation team on a Part‑Time or Per‑Diem basis. What You'll Do Evaluate residents and develop individualized treatment plans Provide skilled physical therapy services to post‑acute and long‑term care residents Collaborate with physicians, nursing staff, occupational and speech therapists, and families Monitor patient progress and modify treatment plans as needed Maintain accurate and timely documentation Educate residents and caregivers on exercises, safety, mobility, and discharge recommendations Help residents achieve their highest level of independence and quality of life Qualifications Doctor of Physical Therapy (DPT) or equivalent degree from an accredited program Current California Physical Therapist license in good standing Strong clinical assessment and treatment skills Excellent communication and teamwork abilities Preferred Qualifications Experience in Skilled Nursing, Post‑Acute, Rehabilitation, or Long‑Term Care settings Knowledge of Medicare documentation and reimbursement guidelines Experience treating orthopedic, neurological, and medically complex patients Why Citrus Heights Post Acute?
