CASE MANAGER (RN) - FULL TIME

Case Manager

Under the direction of the Director, Case Management, this position is responsible to collaborate with patients, patient family members, physicians and other healthcare professionals to design, implement, and evaluate plans of care specific to each patient's needs, maintaining high quality, cost effective patient care.

Qualifications:

Education - Per RN licensure requirements. Bachelor's Degree preferred.

Experience - At least three (3) years previous clinical and/or case management/utilization management experience preferably in a high volume acute care hospital/healthcare facility.

Licenses - Valid, current RN state license or compact.

Able to effectively communicate with key community stakeholders in order to appropriately assist patients.
