Key Responsibilities

• Provide direct physical therapy services to patients in an outpatient clinical setting
• Evaluate, assess, and develop individualized treatment plans based on patient needs and goals
• Deliver hands-on therapeutic interventions and exercise programs to promote patient recovery
• Maintain accurate and timely clinical documentation in accordance with facility and regulatory requirements
• Collaborate with interdisciplinary care teams to ensure coordinated and effective patient outcomes

Required Qualifications

• Active Physical Therapist license in the state of Oregon (permanent license required; compact license alone is not sufficient for this assignment)
• Prior experience working in an outpatient physical therapy setting
• Ability to meet credentialing and contract quality assurance requirements prior to start
• Completion of any required facility-specific documentation outside of standard onboarding clearance

Preferred Experience / Skills

• Experience working in a high-volume outpatient rehabilitation environment
• Familiarity with outpatient orthopedic or sports rehabilitation protocols
