NOW HIRING: Service Coordinator - Home Care Crown Heights, Brooklyn $50000-$60000 + Monthly Bonus Monday-Friday | 9 AM-5 PM Responsibilities Coordinate services and community resources for clients Communicate with clients, families, and service providers Manage and monitor an assigned caseload Conduct quarterly home visits to assess client needs Maintain accurate documentation and required reports Follow up to ensure services are being delivered properly Qualifications Associate or bachelor's degree required Strong communication and people skills Organized, responsible, and detail-oriented Able to manage multiple clients and priorities Strong follow-through and documentation skills Comfortable working independently and as part of a team Benefits Monday-Friday schedule Paid yom tov off Supportive team environment Growth and advancement opportunities #J-18808-Ljbffr Unclassified
