Opportunities and schedules include:Hospice Home Care Registered Nurse (RN) Monday–Friday | 8:00 AM–4:00 PM | Holiday rotationProvide hospice and palliative nursing care to patients in their homes, helping them maintain their independence and quality of life while receiving care in a familiar environment.Facility-Based Services Registered Nurse (RN) Monday–Friday | 8:00 AM–4:00 PM | Holiday rotationProvide individualized hospice nursing care and support to patients and families in facility-based settings. Collaborate with facility staff and interdisciplinary care teams to address patient needs and ensure continuity of care.ResponsibilitiesResponsibilities will vary depending on the Registered Nurse position and care setting and may include:Provide compassionate, individualized nursing care to hospice and palliative care patients.Perform comprehensive assessments addressing physical, psychosocial, environmental, safety, emotional, and spiritual needs.Assess and manage pain and other symptoms associated with serious and terminal illness.Develop, initiate, and update individualized plans of care based on patient needs.Recognize changes in patient condition and provide timely and appropriate clinical interventions.Educate and support patients, families, and caregivers throughout the hospice care process.Collaborate with physicians, facility staff, and interdisciplinary team members to coordinate patient care.Evaluate patient needs, goals of care, and appropriate hospice services, including facilitating admissions when applicable.Maintain accurate and timely electronic medical records and clinical documentation.Follow established clinical, patient safety, privacy, and infection-control standards.Travel to patient homes and/or healthcare facilities as required by the position.QualificationsGraduate of an accredited school of nursing.Current Registered Nurse (RN) license in the State of New York.Valid driver's license and access to a vehicle for community travel.One year of professional nursing experience preferred for Home Care and Facility-Based Services RN positions.Two years of professional nursing experience preferred for the Community Admissions RN position.Certification in Hospice and Palliative Care Nursing preferred.About Hospice and Palliative Care BuffaloThe majority of our patients prefer to receive hospice and palliative care in a home environment where they can maintain their independence and spend meaningful time with family and friends.As a Registered Nurse with Hospice and Palliative Care Buffalo, you'll have the opportunity to provide individualized care and build meaningful relationships with patients and families while working alongside an interdisciplinary clinical team.With multiple Registered Nurse positions available, candidates interested in hospice home care nursing and facility-based nursing are encouraged to apply.Apply today to learn more about our current Registered Nurse (RN) opportunities in Buffalo, NY.
