Physical Therapist (PT)

Position Overview

A Physical Therapist is needed for a 13-week assignment covering full-time 5×8s, Monday-Friday, 8 AM-4:30 PM . This role supports a skilled nursing facility (SNF) and may flex to inpatient acute care based on departmental needs. The PT will treat rehab patients, long-term residents, and may assist on the acute side when cross-trained. Caseload may reach up to 10 patients/day with non-templated scheduling.

• Charting Systems: EPIC & Therapute (rehab patients)

• Interview: Manager calls candidates directly

• Shift Cancellation Policy: N/A

• Caseload: Up to 10 patients/day; SNF + inpatient flex

Responsibilities

• Provide PT interventions for SNF rehab patients and long-term residents

• Flex to inpatient acute care as needed

• Collaborate with interdisciplinary teams and OT

• Contribute to discharge planning and recommendations

• Attend weekly patient care conferences

• Document care in EPIC and Therapute

• Manage non-templated daily caseload with variable visit lengths

Required Experience & Certifications

• Preferred: 1-2 years SNF and rehab experience

• Active PT license (California)

Compensation

Local Physical Therapist

Weekly Pay Package: $2,800/week Hourly Pay: $70/hr

The Physical Therapist is responsible for performing patient evaluations, as well as providing and documenting therapy services in accordance with the plan of care developed for each individual patient and physician's orders when applicable.

Minimum Requirements:

• Current licensure as a Physical Therapist in the state of practice

• Graduate of an accredited school of Physical Therapy

• One (1) year of prior professional Physical Therapy experience preferred

• Current CPR if applicable

• TB Questionnaire, PPD or chest x-ray if applicable

• Current Health certificate (per contract or state regulation)

• Must meet all federal, state and local requirements

• Must be at least 18 years of age
